A recurring question among fans is why this Starfleet officer, despite serving diligently for seven years as the chief medical officer, seemingly lacked a definitive rank insignia on his Doctor Uniform. While Lieutenant Commander rank is denoted by two gold and one black pip, the Doctor was often depicted with three black pips, or sometimes none at all, creating an intriguing inconsistency.
