Orchard Park Family Practice Doctors: Leading Patient-Centered Care in Western NY

Orchard Park Family Practice P.C. is proud to be recognized at the highest level by the National Committee for Quality Assurance (NCQA) for Physician Practice Connections-Patient Centered Medical Home. This prestigious recognition highlights our commitment to providing exceptional, patient-centered care to families in Orchard Park and the wider Western New York region. Our team of dedicated Orchard Park Family Practice Doctors and healthcare professionals are committed to creating a health care setting that truly partners with you, your family, and your personal physician.

What is a Patient-Centered Medical Home?

The Patient Centered Medical Home (PCMH) model is at the heart of our practice. It’s a transformative approach to primary care that focuses on building strong partnerships between patients, their families (when appropriate), and their healthcare team. At Orchard Park Family Practice, we leverage registries, advanced information technology, and health information exchange to ensure you receive the right care, at the right time, and in the most convenient way possible. We strive to deliver care that is not only effective but also culturally and linguistically appropriate for each individual.

The PCMH concept allows our Orchard Park family practice doctors to move beyond simply treating illnesses and injuries as they arise. Instead, we focus on building long-term, coordinated care relationships with our patients. This proactive approach emphasizes preventative care and comprehensive healing, fostering a sense of trust and security within our practice. Knowing that your Orchard Park family practice doctors are consistently making the extra effort for your ongoing health and wellness provides peace of mind for you and your loved ones.
